# Volunteer Signup Form

> Recruit volunteers for an event or organization by collecting availability, skills, and preferences.

## What this template is for

The Volunteer Signup Form is built for non-profit coordinators, event managers, and community organizers who need to recruit and organize volunteers. It collects personal details, skills and availability, previous volunteer experience, emergency contact information, health disclosures, and a signed volunteer agreement. Conditional logic handles guardian information for minors and medical details for those who need to disclose health conditions.

## Suggested Airtable fields

You will want a table with fields covering:

* **Personal info** -- full name, email, phone, and age range
* **Minor status** -- a checkbox indicating if the volunteer is under 18, with guardian name, phone, and email fields
* **Skills** -- a multi-select covering event setup, registration, hospitality, AV/tech support, first aid, photography, and more
* **Availability** -- a multi-select for specific shifts (Friday setup, Saturday morning, Saturday afternoon, etc.)
* **Hours per day** -- a number field for maximum daily hours
* **Experience** -- checkbox for previous volunteering and a text field for details
* **Motivation** -- a text field for why they want to volunteer
* **Emergency contact** -- name and phone
* **Health** -- checkbox for medical conditions and a text field for details
* **Merchandise** -- t-shirt size
* **Agreement** -- terms acknowledgement checkbox and signature field

## Form structure

This template uses **5 pages**:

1. **About You** -- Name, email, phone, age range, and a minor indicator. If the volunteer is under 18, guardian contact fields appear conditionally.
2. **Skills & Availability** -- Skills as checkbox cards with an "Other" option, shift availability as checkbox cards, maximum hours per day, t-shirt size, previous experience checkbox (with conditional detail field), and motivation.
3. **Emergency & Health** -- Emergency contact name and phone. Medical conditions checkbox with conditional detail field for disclosures.
4. **Volunteer Agreement** -- Terms and conditions text block, required acknowledgement checkbox, and signature widget (draw or type).
5. **Thank You** -- Personalized confirmation that the application is under review.
