What this template is for

The Conference Registration Form is designed for conference organizers, event management companies, and professional associations running multi-day events. It handles the full registration flow: attendee information, membership verification, ticket tier selection, workshop add-ons, dietary and accessibility preferences, and payment method selection. Conditional logic adapts the form based on membership status and payment method choice.

Suggested Airtable fields

You will want a table with fields covering:
  • Attendee details — full name, email, phone, organization, job title, and country
  • Membership — a checkbox for existing members and a text field for membership number
  • Ticket type — a single select with tiers like General Admission, VIP, Student, Exhibitor, Speaker
  • Workshops — a linked record field pointing to a Workshops table with capacity and scheduling info
  • Dietary restrictions — a multi-select for common dietary needs
  • Accessibility needs — a long text field for accommodation requests
  • Payment — payment method selector (Credit Card, Bank Transfer, Invoice, Voucher Code), voucher code field, and payment proof file upload
  • Special requests — a catch-all text field

Form structure

This template uses 5 pages:
  1. Attendee Information — Contact details, organization, country, and membership verification. If the attendee checks the membership box, a membership number field appears for discount eligibility.
  2. Ticket & Workshops — Ticket type as radio cards with a description explaining access levels. Workshop selection from linked records (optional, limited to 5 selections). An info block notes that workshops have limited seating.
  3. Preferences & Needs — Dietary restrictions as checkbox cards, plus collapsible groups for accessibility accommodations and special requests.
  4. Payment — Payment method as radio cards with conditional content: voucher code field for voucher payments, bank transfer instructions text block, invoice confirmation text block, and payment proof upload for bank transfers.
  5. Thank You — Confirmation with ticket type, plus a downloadable registration receipt.
