What this template is for

The Vendor Registration Form is built for procurement managers, finance teams, and operations staff who need to onboard new suppliers. It collects everything from legal business details and service categories to certifications, banking information, and an authorized signature — all in a single, structured flow that replaces back-and-forth emails and scattered spreadsheets.

Suggested Airtable fields

You will want a table with fields covering:
  • Business details — legal business name, trading name, business type, year established, registration number, and tax ID
  • Primary contact — name, title, email, and phone
  • Address — street address, city, state/province, postal code, and country
  • Services — service categories as a multi-select (IT Services, Consulting, Manufacturing, etc.) and a description field
  • Company size — annual revenue range and employee count
  • Certifications — multi-select for ISO, SOC 2, GDPR, minority/women/veteran-owned, etc.
  • Insurance and licensing — checkboxes for current certificates
  • Supporting documents — file upload for W-9, insurance certificates, licenses
  • Banking — bank name, account holder, routing number, account number, payment method, and payment terms
  • Authorization — signatory name and signature field

Form structure

This template uses 5 pages:
  1. Business Information — Legal details (business name, trading name, type, year established, registration and tax numbers) and primary contact information.
  2. Address & Services — Business address with autocomplete, service categories as checkbox cards, services description, and company size selectors.
  3. Credentials & Documents — Certification multi-select, insurance and license checkboxes, and a file upload for supporting documentation.
  4. Banking & Payment — Bank account details with masked account number input, payment method and terms as radio cards, a confidentiality notice, authorized signatory name, and a signature widget.
  5. Thank You — Confirmation with vendor name and contact email, noting the review timeline.
