What this template is for
The Testimonial Collection Form is built for marketing teams, customer success managers, and solopreneurs who want to systematically collect social proof from happy customers. It goes beyond a simple “leave a review” box by gathering a punchy headline, a detailed testimonial, specific outcomes, a professional headshot, and granular publication consent (name, company, and photo each controllable separately). The result is testimonial content that is ready to publish on your website, social media, or marketing materials.Suggested Airtable fields
You will want a table with fields covering:- Person details — full name, job title, company, industry, email, and LinkedIn URL
- Customer context — how long they have been a customer and which product or service they used
- Testimonial content — overall rating (1-5), a short headline, the full testimonial as rich text, and a specific outcome or result
- Photo — a file upload for a professional headshot
- Publication consent — individual checkboxes for allowing name, company name, and photo to be used publicly, plus a required overall consent confirmation
- Status — for tracking new, approved, published, and rejected testimonials
Form structure
This is a single-page form with a thank-you page:- Your Testimonial — Personal details (name, job title, company, industry, email, LinkedIn), customer context (how long a customer, product used), overall rating, headline (short and punchy, max 120 characters), full testimonial in rich text, specific outcome or result, optional headshot upload (images only, max 5 MB), and publication consent section with individual toggles and a required confirmation checkbox. A text block explains what the consent controls.
