What this template is for

The Wedding RSVP Form gives couples and wedding planners a beautiful, easy-to-use way for guests to confirm attendance, select their meal, indicate dietary restrictions, and add plus-one details. It uses conditional logic extensively — guests who decline see a minimal form, while attending guests are walked through meal selections, plus-one information, and a song request. The styling is intentionally elegant with a soft botanical theme.

Suggested Airtable fields

You will want a table with fields covering:
  • Guest details — guest name, email, and phone
  • Attendance — a single select with options like “Attending with joy” and “Regretfully unable to attend”
  • Meal choice — a single select for entree options (e.g., Beef Tenderloin, Pan-Seared Salmon, Mushroom Risotto)
  • Dietary restrictions — a multi-select for common dietary needs, plus a text field for additional notes
  • Plus one — a checkbox for bringing a guest, plus-one name, plus-one meal choice, and plus-one dietary restrictions
  • Fun extras — a song request text field and a message for the couple

Form structure

This is a single-page form with a thank-you page:
  1. RSVP — Opens with an image widget (couple photo placeholder) and a text block with event details (date, venue). Guest name, email, and phone. Attendance selection as radio cards. If attending: meal choice as radio cards, dietary restrictions as checkbox cards (with a conditional notes field for allergies), plus-one checkbox with conditional fields for the guest’s name, meal, and dietary restrictions, and a song request field. A message for the couple is always visible regardless of attendance.
  2. Thank You — Personalized confirmation that the RSVP was received.

Styling

This template uses a custom romantic theme: rose/pink accent color, Quicksand font, soft pink background, pill-shaped buttons. A soft botanical illustration is recommended as a full background image. This is the most visually distinctive template in the library.
