What this template is for

The Event Registration Form is built for event coordinators, community managers, and workshop facilitators who need to register attendees and collect their preferences. It gathers contact details, attendance type (in-person, virtual, or hybrid), session selections, dietary restrictions, accessibility needs, and emergency contact information. Conditional logic keeps the form lean — virtual attendees skip questions about t-shirt sizes and emergency contacts that only apply to in-person attendance.

Suggested Airtable fields

You will want a table with fields covering:
  • Contact details — full name, email, phone, organization, and job title
  • Attendance type — a single select for In-Person, Virtual, or Hybrid
  • Sessions — a linked record field pointing to an Event Sessions table
  • Dietary restrictions — a multi-select with common dietary needs (Vegetarian, Vegan, Gluten-Free, Halal, etc.)
  • Dietary notes — a text field for additional detail on allergies or restrictions
  • Accessibility — a checkbox and a text field for specific accommodation needs
  • Merchandise — t-shirt size selector
  • Referral source — how the attendee heard about the event
  • Emergency contact — name and phone number

Form structure

This template uses 4 pages:
  1. Your Information — Contact details and attendance type selection as radio cards.
  2. Sessions & Preferences — Session selection from linked records (shown for in-person and hybrid attendees), dietary restrictions as checkbox cards, optional dietary notes (shown when “Other” is selected), t-shirt size (shown for in-person and hybrid), accessibility needs checkbox with conditional detail field, and referral source.
  3. Emergency Contact — Emergency contact name and phone, shown conditionally for in-person and hybrid attendees.
  4. Thank You — Confirmation message with registration details, plus an option to register another person.

Styling
