What this template is for
The Lead Capture Form is designed for marketing teams, growth teams, and sales reps running inbound campaigns. It uses a short, two-step flow to capture contact information and then qualify the lead with company size, industry, primary challenge, current tools, and buying timeline. The form blocks free email providers to ensure you are collecting business leads, and uses progressive disclosure to keep the experience fast while still gathering qualification data.Suggested Airtable fields
You will want a table with fields covering:- Contact info — first name, last name, work email, company, and job title
- Company profile — company size and industry
- Qualification — primary challenge (a single select with options like “Managing data in Airtable,” “Building client-facing forms,” etc.), a details field for custom challenges, and current tools in use
- Buying signal — how soon the prospect is looking to get started (Immediately, Within 1 month, etc.)
- Source tracking — lead source (Google, LinkedIn, Referral, etc.)
- Consent — a checkbox for agreeing to be contacted
- Lead scoring — a number field for automated scoring based on qualification data
Form structure
This template uses 3 pages:- Get Started — First name, last name, work email (with free email provider blocking and typo detection), company, and job title. Minimal friction to get the lead into the funnel.
- Your Situation — Company size as radio cards, industry dropdown, primary challenge as radio cards, conditional details field (shown when “Other” is selected), current tools text field, and buying timeline as radio cards.
- Thank You — Personalized message with a redirect to the signup page.
