What this template is for

The Product Order Form is designed for e-commerce operators, merchandise sellers, and small business owners who want customers to place orders through a structured form that syncs directly to Airtable. It covers product selection from a linked catalog, quantity and sizing, shipping details with address autocomplete, gift options, and promo codes. Conditional logic hides the shipping address for local pickup orders and shows gift message fields only when needed.

Suggested Airtable fields

You will want a table with fields covering:
  • Customer info — name, email, and phone
  • Order details — linked record for products (filtered to in-stock items), quantity, size, color preference, and order notes
  • Shipping — shipping method (Standard, Expedited, Overnight, Local Pickup), address fields (line 1, line 2, city, state/province, postal code, country)
  • Gift options — a checkbox for whether the order is a gift and a text field for a gift message
  • Discounts — promo code text field
  • Special instructions — a text field for delivery notes
  • Order tracking — auto-number order number and status field

Form structure

This template uses 3 pages:
  1. Your Order — Customer name, email (with typo detection), and phone. Product selection from linked records (filtered to in-stock items, up to 20 selections). Quantity, size dropdown, color preference, and order notes.
  2. Shipping — Shipping method as radio cards. Conditional shipping address fields (hidden for local pickup; a text block with pickup location instructions appears instead). Gift checkbox with conditional gift message field. A collapsible group for promo code and special delivery instructions.
  3. Thank You — Order confirmation referencing the customer name, email, and shipping method. Includes a downloadable order summary and an option to place another order.
