What this template is for

The Project Request Form gives operations managers, project coordinators, and team leads a structured way to collect incoming work requests. Whether the request comes from another department or directly from a client, this form captures the project scope, priority, timeline, and any relevant attachments so the receiving team can triage and plan effectively.

Suggested Airtable fields

You will want a table with fields covering:
  • Requester details — name, email, and department
  • Project overview — project name, project type (New Feature, Bug Fix, Campaign, etc.), and priority level (Critical, High, Medium, Low)
  • Description and goals — long text fields for describing the work and defining success criteria
  • Timeline — start date and deadline date fields, plus an estimated effort selector (less than 1 day through 1+ months)
  • Stakeholders — a linked record field pointing to a contacts table
  • Attachments — a file upload field for briefs, references, or supporting documents
  • Special requirements — a long text field for constraints or dependencies

Form structure

This template uses 3 pages:
  1. Request Details — Requester info (name, email, department), project name, type and priority as radio cards, project description, and goals. An informational text block at the top explains the review timeline.
  2. Scope & Timeline — Start date and deadline fields, estimated effort as radio cards, stakeholder selection via linked records, file attachments, and a collapsible group for special requirements.
  3. Thank You — Personalized confirmation referencing the project name and requester.
