What this template is for
The Post-Event Feedback Form helps event coordinators, conference organizers, and community managers collect actionable feedback from attendees after an event. It covers overall impressions, session and speaker feedback, and tailored questions based on whether the attendee was in-person or virtual. In-person attendees rate the venue and networking opportunities, while virtual attendees rate the streaming experience and can report technical issues. This split ensures every attendee gets relevant questions.Suggested Airtable fields
You will want a table with fields covering:- Attendee info — name and email (both optional)
- Attendance type — In-Person or Virtual
- Overall ratings — overall event rating, organization rating, and content quality rating (all 1-5)
- In-person ratings — venue rating and networking opportunities rating
- Virtual ratings — virtual experience rating, plus a checkbox and text field for technical issues
- Session feedback — linked records for sessions attended, favorite session text field, and speaker feedback
- Open-ended — what worked well, what could improve, and additional comments
- Likelihood — would attend again and would recommend (both single selects)
Form structure
This template uses 4 pages:- Your Experience — Optional name and email. Attendance type as radio cards. Overall event, organization, and content quality ratings. Conditional in-person fields (venue and networking ratings) or virtual fields (virtual experience rating, technical issues checkbox with conditional details field).
- Sessions & Speakers — Sessions attended from linked records, favorite session, and speaker feedback text.
- Final Thoughts — What worked well, what could improve, would attend again and would recommend as radio cards, and additional comments.
- Thank You — Simple thank-you message emphasizing that feedback shapes future events.
