What this template is for

The Maintenance Request Form is built for facilities managers, property managers, and operations teams who need a structured way to receive and triage repair requests. Employees or tenants submit the issue category, urgency level, a detailed description, their location in the building, optional photo evidence, and contact preferences. The form is a single page for speed — when something is broken, you do not want the requester navigating through multiple steps.

Suggested Airtable fields

You will want a table with fields covering:
  • Requester info — name, email, phone, and department
  • Issue details — issue category (HVAC, Plumbing, Electrical, IT/Equipment, Security, etc.), urgency level (Emergency, High, Medium, Low), and a detailed description
  • Location — building/floor selector, a text field for exact location details, and an optional GPS coordinate field
  • Evidence — a checkbox for whether photos are available and a file upload field
  • Asset identification — a barcode field for scanning equipment tags
  • Contact preferences — preferred contact method (Email, Phone, Either) and best time to contact
  • Access instructions — a text field for how to reach the affected area
  • Resolution tracking — assigned technician, resolution notes, status, and resolved date

Form structure

This is a single-page form with a thank-you page:
  1. Maintenance Request — Requester name, email (with typo detection), phone, and department. Issue category and urgency as radio cards (with a note that emergencies are escalated immediately). Detailed issue description with minimum length requirement. Building/floor dropdown and exact location text field. Optional GPS location capture widget. Photo checkbox with conditional file upload (images only, up to 10 files). Barcode field for asset tag scanning. Contact method as radio cards and best time dropdown. A collapsible group for access instructions.
  2. Thank You — Confirmation with the requester’s name, plus a reminder to call facilities directly for urgent safety issues.
