What this template is for

The Price Quote Request Form lets sales teams, e-commerce operators, and product businesses collect structured quote requests from potential customers. Instead of fielding vague email inquiries, this form gathers the quote type, product selections, quantities, budget, delivery needs, and any special instructions — giving your sales team everything they need to respond with an accurate quote quickly.

Suggested Airtable fields

You will want a table with fields covering:
  • Contact info — name, company name, email, and phone
  • Quote type — a single select for the kind of request (Product Purchase, Custom Services, Bulk Order, Subscription, etc.)
  • Products or services — a linked record field pointing to a products catalog table
  • Custom item description — a long text field for describing custom or non-catalog requests
  • Quantity and budget — a number field for units and a currency field for target budget
  • Currency preference — a selector for USD, EUR, GBP, etc.
  • Delivery details — delivery address and required-by date
  • Special instructions and attachments — long text and file upload for specs or references

Form structure

This template uses 3 pages:
  1. Contact Details — Name, company, email, and phone with typo detection on the email field.
  2. Quote Details — Quote type as radio cards, then conditional fields: product selection from a linked catalog (for product/bulk orders), or a free-text description field (for custom services). Quantity, target budget, currency, delivery date, and delivery address (shown only for product purchases and bulk orders). A collapsible group at the bottom holds special instructions and file attachments.
  3. Thank You — Personalized confirmation promising a quote within 24 hours.
