What this template is for

The Employee Onboarding Form helps HR teams and people operations managers get everything they need from a new hire before their first day. It covers personal details, emergency contacts, equipment preferences, payroll and tax information, identity document uploads, and a handbook acknowledgement with signature. The goal is to eliminate the usual back-and-forth of onboarding paperwork by collecting it all in one clean flow.

Suggested Airtable fields

You will want a table with fields covering:
  • Personal details — full legal name, preferred name, personal email, phone, date of birth, and start date
  • Home address — street address, city, state, and zip code
  • Emergency contact — contact name, relationship, phone, and email
  • Equipment — computer preference, desk setup preference, and t-shirt size for a welcome kit
  • Tax information — filing status and withholding allowances
  • Direct deposit — checkbox for opting in, plus bank name, account type, routing number, and account number
  • Identity documents — file upload for government-issued ID and work authorization
  • Acknowledgement — handbook checkbox, signature field, and optional notes

Form structure

This template uses 6 pages:
  1. Personal Information — Legal name, preferred name, personal email, phone, date of birth, start date (read-only), and home address with autocomplete.
  2. Emergency Contact — Emergency contact name, relationship, phone, and email.
  3. Equipment Preferences — Computer preference and desk setup as radio cards, t-shirt size as a button group.
  4. Payroll & Tax — Tax filing status, withholding allowances, direct deposit opt-in (with conditional fields for bank details), and identity document upload. A confidentiality notice is shown as an info block.
  5. Acknowledgement & Signature — Handbook acknowledgement text block, required checkbox, signature widget (draw or type), and a collapsible group for additional notes.
  6. Thank You — Personalized welcome message using the new hire’s preferred name and start date.
