What this template is for
The Employee Self-Evaluation Form is designed for HR managers and people operations teams running performance review cycles. Employees use this form to reflect on their accomplishments, rate themselves across key competencies, set goals for the next period, and identify development needs. The multi-page structure breaks the evaluation into manageable sections so employees can give thoughtful responses rather than rushing through a single long form.Suggested Airtable fields
You will want a table with fields covering:- Employee info — name, email, job title, department, manager name, and review period
- Reflections — rich text fields for key accomplishments, challenges faced, and goals for the next period
- Competency ratings — rating fields (1-5 scale) for areas like job knowledge, communication, teamwork, problem solving, initiative, and leadership
- Competency comments — a text field paired with each rating for written context
- Overall assessment — an overall self-rating and overall comments field
- Development — text fields for training needs and career goals
- Manager feedback — a text field where the employee can request specific feedback from their manager
Form structure
This template uses 4 pages:- About This Review — Employee info (name, email, job title, department, manager, review period) and three rich text reflection fields: key accomplishments, challenges faced, and goals for the next period. An instructional text block sets the tone for honest, specific answers.
- Competency Ratings — Six competency sections (Job Knowledge, Communication, Teamwork, Problem Solving, Initiative, Leadership), each with a rating field and an optional comments field. A text block at the top explains the 1-5 rating scale.
- Overall & Development — Overall self-rating with required comments, training needs, career goals, and a collapsible group for requesting specific manager feedback.
- Thank You — Confirmation that the evaluation was submitted, noting the manager will review and schedule a meeting.
