What this template is for

The Demo Request Form is designed for sales teams and product-led growth teams handling inbound demo requests. It walks prospects through providing their contact and company information, describing their use case and timeline, and then scheduling a call directly via an embedded calendar widget. The form blocks free email providers and collects enough qualification data to let your sales team prepare a tailored demo.

Suggested Airtable fields

You will want a table with fields covering:
  • Contact info — first name, last name, work email, phone, company, and job title
  • Company profile — company size and industry
  • Use case details — whether they are a current Airtable user, primary use case (Client-Facing Forms, Internal Process Automation, Document Generation, etc.), additional details, team size, and buying timeline
  • Consent — a checkbox for agreeing to communications
  • Status tracking — for Requested, Scheduled, Completed, No Show, Cancelled

Form structure

This template uses 4 pages:
  1. Book a Demo — First name, last name, work email (with free email blocking and typo detection), company, job title, company size as radio cards, and industry dropdown.
  2. About Your Use Case — Airtable usage toggle (button group), primary use case as radio cards, optional details text field, team size dropdown, buying timeline dropdown, and a required consent checkbox.
  3. Schedule — An embedded calendar widget (Calendly or similar) where the prospect picks a time slot. The page description references the prospect’s email dynamically.
  4. Thank You — Personalized confirmation that the demo is booked, with a note to check their email for the calendar invite.
