What this template is for

The Job Application Form is designed for HR managers, talent acquisition teams, and hiring managers who need a polished, multi-step application process. Candidates walk through providing their contact details, selecting the position they are applying for, sharing their work history and education, uploading a resume, answering screening questions, and optionally providing references. The form connects to an Airtable base with a linked Job Openings table for position selection.

Suggested Airtable fields

You will want a table with fields covering:
  • Contact details — full name, email, phone, LinkedIn URL, and portfolio/website URL
  • Position — a linked record field pointing to a Job Openings table (filtered to show only open positions)
  • Preferences — employment type (Full-Time, Part-Time, Contract, etc.) and work location preference (Remote, On-site, Hybrid)
  • Experience — years of experience, current employer, current title
  • Education — highest education level and field of study
  • Application materials — resume file upload, relevant skills as a multi-select, a “why this role” short answer, and optional cover letter
  • Availability — start date and salary expectation
  • Sponsorship — checkbox for visa sponsorship requirement
  • References — checkbox for availability, plus a long text field for reference details
  • Source — where the candidate found the job posting

Form structure

This template uses 4 pages:
  1. Personal Information — Contact details (name, email, phone) and online presence links (LinkedIn, portfolio).
  2. Position & Experience — Position selection from linked records, employment type and location preference as radio cards, work history (current employer, title, years of experience), and education fields.
  3. Application — Resume upload (documents only, max 10 MB), skills as checkbox cards with an “Other” option, screening questions (why this role, optional cover letter), availability and salary, sponsorship requirement, and a collapsible references section with conditional logic.
  4. Thank You — Personalized message with expected response timeline.
